Video Release Form

By completing the information below and selecting the button below marked “I AGREE” I grant permission to ISSA and all its divisions to the right to use, edit, copy, enhance, exhibit, display and distribute: i) my image including video, electronic, photographic, printed and motion picture images of me; and ii) my voice including sound and audio recordings of me for any lawful purpose in any and all media and waive any right to any compensation, including royalties, arising from the use of the materials described in (i) and (ii) (the “Materials”).  The permission granted herein shall extend to all formats, languages, and other forms of media known or henceforth created; and the term of this authorization and permission shall remain active unless revoked in writing. 

I understand and agree that the Materials shall become the sole property of ISSA, and shall not be returned to me.  I understand that my image may be edited, enhanced, copied, exhibited, displayed or distributed and waive any right to inspect or approve any such finished product in which my image appears.
